Athletic Physicals

Athletic physicals will be offered on May 4, 2011 at Skyline Medical Center from 5:00-7:30pm.   All students that will be in 8-12th grade during the 2010-11 school year and plan to participate in EPS athletics will be required to turn in a physical dated after 5/1/10. Skyline Medical Center is located at 1908 N. 203rd St., Suite 2, Elkhorn, NE   68022

You must have a physical turned in before you will be allowed to participate in conditioning.

Please bring $20.00 (Cash only) to the physical.  The forms are available on the school website or you may fill them out when you arrive for your physical.
